Direct answer: the cheapest FDA-approved option is $149/month — Foundayo (oral) via LillyDirect, or the oral Wegovy tablet via NovoCare. The cheapest compounded options are NexLife's semaglutide microdose at $110 and Oak Longevity's semaglutide at $133
Necessary qualification: an FDA-approved oral pill at $149 now undercuts most of the compounded market, so 'compounded is cheaper' is no longer reliably true. Microdose programmes sit below every dose studied in the trials. Committed-plan rates are not month-to-month rates, and introductory rates are not ongoing rates — we keep all of these separate

The three rules we sort by

1. Total, not medication. Every figure below is medication plus any membership you cannot decline. Eden quotes $99 for compounded semaglutide; the membership is required, so the number here is $198.

2. Ongoing, not introductory. Eleven of these offerings are first-month rates. They are flagged and excluded from every ranking.

3. Monthly equivalent = plan total ÷ plan months. Where a price depends on a commitment, we publish the plan total beside the monthly figure so the arithmetic is checkable.

Why we label prices instead of just listing themEvery price in this database was captured from the provider's own published pages and confirmed on July 6, 2026. That is what our Verified label means: a dated capture against the source, not a figure copied from another comparison site.

The distinction matters, because comparison sites in this category publish flatly contradictory numbers for the same providers. Our own capture corrected two of them: a stored TrimRx figure of $259 that matched no current checkout tier, and an Eden brand-Zepbound figure of $299 that turned out to be LillyDirect's price, not Eden's — Eden's real price is $1,399.

Prices change. This capture is dated, and a figure more than about thirty days old should be confirmed at checkout. What we will not do is mark something Verified because someone else published it.

A capped dose is not a discountWatch for dose caps as well as dose escalation. Noom Med's $199 compounded semaglutide programme is capped at 0.6mg — the STEP trials that established semaglutide's efficacy used 2.4mg. A capped programme is not a cheaper version of the same treatment; it is a lower-dose treatment, and the expected effect is correspondingly smaller. Noom's full-titration programme is $279.

Compounded liraglutide — 2 offerings

Liraglutide is a daily injection rather than weekly, which is the main practical difference. It is also meaningfully less effective: SCALE found roughly 8% mean weight reduction, against 14.9% for semaglutide in STEP 1 and 20.9% for tirzepatide in SURMOUNT-1. It is rarely the right choice on effectiveness, and it is not cheaper.

An FDA-approved, quality-verified, manufacturer-supplied medication at $149, against a compounded market that mostly runs $169-$399. The catch is dose escalation — Foundayo rises to $199, then $299, then $349 as you titrate, and at the top doses it has its own 45-day refill rule (it drops back to $299 if you refill in time). But for a starting patient, or anyone maintaining on a lower dose, the brand oral pill is now among the cheapest legitimate options in the entire category — and almost no comparison site has caught up.

The retail trap

Check the manufacturer before you buy brand through a platformSeven offerings in this database are brand-name drugs resold at close to retail, while the manufacturer sells the identical medicine direct for a fraction of the price. Eden lists brand Wegovy at $1,794/month total; NovoCare sells it for $149-$349. Eden lists brand Zepbound at $1,498; LillyDirect sells it for $299-$449. Hers lists Mounjaro at $2,048.

These are not scams — the prices are disclosed. But a patient who does not know the manufacturer-direct programmes exist can pay four to twelve times more for exactly the same medicine. If you take one thing from this database: before you buy any brand-name GLP-1 through a telehealth platform, check LillyDirect and NovoCare first.

Never pay cash for MounjaroMounjaro and Zepbound are the same molecule — tirzepatide. The only difference is the approved indication: Mounjaro for type 2 diabetes, Zepbound for weight management.

Lilly runs a self-pay programme for Zepbound and none for Mounjaro. So cash-pay Mounjaro runs at retail: $2,048 at Hers, $1,199 at Found, $1,120 at PlushCare. Brand Zepbound through LillyDirect is $299-$449 for the identical drug.

If you are paying cash and you want tirzepatide, you want Zepbound. Mounjaro makes financial sense only when insurance covers it, which generally requires a type 2 diabetes diagnosis.

The intro-rate trapEleven of the eighty-nine offerings in this database are introductory rates — and they are the ones that get quoted. Noom advertises $79; the ongoing price is $199. MEDVi advertises $179; refills are $299. TrimRx's $179 becomes $299. Eden's $39 membership becomes $99. Oak's oral $245 is a four-week supply, which is $266 normalised to a month.

You pay the ongoing rate for eleven of your twelve months. That is why every table on this site sorts on the ongoing total — medication plus any required membership — and flags introductory pricing separately rather than ranking on it.

Split billing

The commonest way a price looks lower than it isRoughly half the offerings here use split billing — a medication price plus a separate membership — and it is the commonest way a price looks lower than it is. Eden quotes $99 for compounded semaglutide, but the membership is required for any medication, so the real price is $198. Hims and Hers quote $149 for the Wegovy pill; with the $149 membership it is $298. Ro quotes $199; it is $348.

A membership is not automatically bad. Mochi's $79 buys unlimited physician and dietitian access. PlushCare's $19.99 buys prior-authorisation support that can save you far more than it costs. The problem is not the fee — it is a comparison table that omits it. Every total in this database includes it.

Delivery formats — and the distinction that actually matters

Approved vs compounded, not oral vs injectableThis database covers three delivery formats, and they are not interchangeable. Injectable is the only format with trial evidence — every SURMOUNT and STEP result is a subcutaneous injection. Oral splits into two very different things: FDA-approved oral products (Foundayo, the Wegovy tablet), which have their own trial programmes, and compounded oral or ODT products, which have none. Sublingual — Shed's lozenges and drops, Henry Meds' sublingual tablets — likewise has no published trial evidence in this class.

The distinction that matters is not injectable-versus-oral. It is approved-versus-compounded. An FDA-approved oral pill has been tested in humans. A compounded sublingual drop has not, and its bioavailability is unpublished.

What a commitment actually costs you

Before you commit to a long planA committed plan lowers the monthly figure and raises the risk. Before you sign one, ask what happens if you stop early — because a meaningful number of people do. Roughly one in five patients discontinues a GLP-1 within the first few months, most often because of gastrointestinal side effects. Others stop because insurance unexpectedly approves a brand product, or because they reach a goal weight, or because their circumstances change.

Providers differ enormously in what happens then. Some refund the unused portion. Some convert you to the month-to-month rate and bill the difference for months already taken. Some refund nothing. This is the single question people most often forget to ask, and it is the one most likely to cost them money.

Dose escalation: the risk the headline price hides

The question that matters more than the headline priceAsk what you will pay at your target maintenance dose, not at the starting dose. This is the difference between a programme that quotes a flat rate at every dose and one that escalates: MEDVi's compounded tirzepatide reaches $499/month at 10-15mg against a $399 headline; Shed's injectables rise with dose; Oak escalates $50-$75 per step. The insurance pathway

Coverage is most common through employer-sponsored commercial plans. Zepbound is excluded from Medicare Part D for weight loss and from most state Medicaid programmes. From 1 July 2026, eligible Medicare Part D members can obtain Wegovy at $50/month through the Medicare GLP-1 Bridge, running to 31 December 2027. Expect prior-authorisation paperwork: typically a BMI of 30+, or 27+ with a weight-related condition.

Dose caps: the other thing a low price can hide

A capped dose is not a discountWatch for dose caps as well as dose escalation. Noom Med's $199 compounded semaglutide programme is capped at 0.6mg — the STEP trials that established semaglutide's efficacy used 2.4mg. A capped programme is not a cheaper version of the same treatment; it is a lower-dose treatment, and the expected effect is correspondingly smaller. Noom's full-titration programme is $279.

How to verify any of this yourself

You should not take our word for a price, and you do not have to. Every figure here can be checked in a few minutes.

  1. Go to the provider's own pricing page. Not a comparison site — the provider's. Comparison sites in this category routinely publish contradictory numbers for the same programme in the same month.
  2. Find the ongoing price, not the headline. Look for the words "first month", "intro", "starting at" or "new patients". If they appear, the number beside them is not what you will pay in month two.
  3. Add the membership. If the medication and the membership are billed separately, add them. That sum is your real monthly cost.
  4. Ask what the highest dose costs. By email or chat, so you have it in writing.
  5. Ask about early cancellation before you commit to a plan longer than a month.
  6. Check the manufacturer. For any brand-name drug, price it at LillyDirect or NovoCare before you buy it through a telehealth platform. Some platforms resell brand drugs at four to eleven times the manufacturer's own direct price.

How many providers and offerings are in this database?

What is the cheapest option in the whole database?

Excluding introductory rates, the cheapest FDA-approved option is Foundayo (oral) or the oral Wegovy tablet at $149/month. The cheapest compounded options are NexLife's semaglutide microdose at $110 and Oak Longevity's semaglutide at $133. But 'cheapest' has at least six distinct meanings and we keep them separate.

Why are some prices so much higher than others for the same drug?

Because some platforms resell brand-name drugs at close to retail. Eden lists brand Wegovy at $1,794/month total; NovoCare sells the identical drug for $149-$349. This is disclosed pricing, not fraud — but it is a four-to-twelve-fold difference for the same medicine.

Are the introductory rates real?

Yes, but they apply to one month. Noom's $79 becomes $199. MEDVi's $179 becomes $299. We flag all eleven intro rates and exclude them from every ranking.
